the point of0:16care ultrasound space with the category0:19defining company itself at the helm so0:22here to tell us all about what's new is0:24the president CEO and chairman of0:27butterfly Network Joe devivo Joe it's0:29great great to have you here it's so0:31happy to be with you nice to see you0:33again Jessica yeah friend of the show0:35Joe we know you from touch and then when0:38InTouch got acquired by teledoc for $6000:41million back at the beginning of 20200:43and so we've got a lot of memory Lan to0:45walk down but before we do that let's0:48dive into the news that's happening0:49today and so butterfly Network just0:52launched its next generation of its0:54handheld ultrasound so this is the0:56butterfly iq3 and the big deal about0:59this is that there's a lot of like sexy1:01Tech that I'm sure we're going to talk1:02through but I'd like to kind of kick off1:04the story and put this in context with1:07something that you said back at JP1:09Morgan this year in 2024 about butterfly1:13Network needing to get its mojo back and1:15returning to revenue growth so I'm sure1:18all of those shareholders who watched1:20the company spack IPO back in 20201 are1:24on the edge of their seat to find out1:26how this brand new version of the1:28handheld ultrasound is going to help you1:30guys capture the momentum you need to1:32chase down that Mojo So Joe tell us all1:35about1:36it well thank you for that Jessica it's1:38you know we're going to have a great1:39year we're going to have a great year1:41you1:42butterfly has a massive Market1:44opportunity in point of care ultrasound1:46and you know the company just went for1:49it last year we had to go through some1:52restructurings and some changes and it's1:55done I've got a great team I've have1:58great technology2:00and now we have a new product that's2:03going to change the way that point of2:05carolan is done so when I say we're2:08going to get our mojo back uh you we2:10ended off uh the end of 23 really strong2:14and I was very pleased with the team I2:15was pleased with the momentum and now2:18with the new products uh it's just going2:20to be a great great year I'm excited to2:22hear a little bit about this so like2:24let's dive into the sexy Tech because2:26like part of this is like this this2:28semiconductor Tech technology that's at2:30the core of this so explain this to me2:32because what I understand is that this2:34really helps the image quality which has2:37been part of the issue with handheld2:39ultrasound so unpack the tech for us and2:41tell us like how good are these images2:43getting I mean are we at the level where2:44we're looking at Cardiology and OB or2:47where are we at here well so yes I I2:50think we are and you know what's2:53exciting is about the rapid evolution of2:57the2:57technology semiconductors have been able3:00to double processing power uh every 183:04months for the last 60 years and we're3:08in a space where we're the only3:10commercially available3:12semiconductor-based ultrasound company3:15and the technology is moving very fast3:18we've gone from our first generation in3:212018 second in 2020 and now here in 20243:25our third and the Leaps and Bounds are3:29sign ific and what's great about3:32semiconductor technology that we're not3:33analog we compete in an analog3:36Marketplace that has battery transducer3:39lens fixed lens and3:41Crystal and and that's the standard piso3:44environment and there and and there's3:46physics involved with how much energy3:49can be delivered through the crystal3:52before there's so much of a heat3:53artifact we're human beings we can only3:55hold something so hot and so um so so3:59while our the Legacy analog competition4:04has reached its limit we have this4:07exponential compute power and so our4:11last our second generation device had a4:14processing power of 4.8 gigahertz per4:18second um this next probe is 9.64:22gahz it's it's like running4:2720,000 4K movies4:30simultaneously that's how much4:32processing power is going through our4:34probe and what that does is allows us to4:38have a lot of frequency a lot of energy4:40and a lot of interpretation of the image4:42to deliver on quality and so4:45historically we we have sold more4:47devices than any of the other companies4:50combined we have placed over a 100,0004:52units in four years and we are all over4:56the world the the if there was a rub if5:00someone said hey this one semiconductor5:02probe that does everything if I can ask5:05anything else of it it would be to5:08improve that image quality to get5:09ourselves to be more comfortable with5:11cardiac imaging um there's so many5:14things that we do well but the processor5:18right now is at a level of capability5:21that we've met that we've met that Mark5:23so it's an incredible time not5:26only for today but it demonstr rates5:30that we can rapidly innovate so we're on5:31our third generation today if we can be5:34equivalent to everybody today what's our5:38fourth generation going to look like in5:39a couple years what's our fifth5:41generation going to look like a few5:42years after so so the future is just5:45amazing I love this you're like leaning5:47hard into Moore's Law here too I mean as5:49far as the pricing on it goes and the5:51fact that this technology is just5:53growing exponentially explain to us the5:55market for this Joe because I think5:57that's one of the questions like all5:58along I mean like like you guys are6:00butterfly network is like category6:01creating like this handheld point of6:04care ultrasound did not exist until6:06ultrasound until butterfly Network6:08brought its ultrasound to Market so it6:10tell us where we're at today like what6:13does this look like in terms of like6:15who's using it I mean is this being6:17adopted like you know the vision was to6:19democratize Medical Imaging and bring it6:21into doctor's offices and Urgent Care6:24and you know even you know different6:25places of the hospital rather than6:27waiting to go and see an ultrasound6:29technician like let's bring it down into6:30the ER so I mean where is it really6:33happening well and and I think that6:35that's the key because there's a6:37difference between market and6:40opportunity Market is the collective6:44revenue of all the companies that are6:46selling a similar product in a similar6:49space the opportunity are all the people6:53who could benefit from the technology6:55but they have to change their behavior6:57in order to benefit from it and so so6:59that's that's you know when you say7:01category leading you know that it's it's7:04you have to be in an environment and in7:07a mindset to change people's behavior7:10and and and I'll give you I'll give you7:11an example like two months7:13ago uh we brought my son to an emergency7:15room he's fine but we were worried at7:17the moment his doctor said hey you7:19really should get him checked so we went7:22to a children's hospital we waited for7:23two hours in a waiting room we got into7:25the room with the7:26doctor the doctor then said uh you know7:30I think it might be this this and that7:31but we have to get confirmation on Ultra7:34on ultrasound excuse me we waited three7:38hours to get an ultrasound they had to7:41wait for the wheelchair and wait for7:43ultrasonographer wait for the7:44radiologist do the read wait for the7:47thing to come back and I said to the to7:49the ER doctor hey I'm CEO a butterfly he7:52goes no way you your7:54butterfly he goes you have to get me a7:57butterfly and I said well why why aren't7:59you using but he goes I just don't know8:01how to use it I don't know how to do8:02ultrasound I don't know how to do the8:04scan yet you have to help me and so we8:06talked about it for a while but had he8:09known how to use it he would have been8:11able to pull it out and within five8:14minutes do the confirmation read and8:18we'd be gone and there was a line of8:19people uh inside the emergency room8:22waiting for the room that we were8:24occupying for three hours so the concept8:26of point of care is to empow each of8:29these nurses there's a million doctors8:31in the US 9 million worldwide 4 million8:34nurses in the US 27 million8:37worldwide and there's also 50 million8:40chronicare patients around the world um8:42that can benefit from this and so the8:44market the opportunity is massive but we8:48have to go through change management so8:50what what are some of the the the change8:53management it it has to do with Comfort8:56level of image acquisition Comfort level8:58in how they would acquire the image and9:01be able to feel like they can make the9:03diagnosis and that's all learning Cur9:06and9:06today 60% of medical schools are9:11educating on9:12butterfly think about that that's9:14awesome there there's an inevitability9:17Factor here right so when you talk about9:19will things happen it's done the D cast9:23the decision is made these kids who come9:26out of school over the next9:28generation are all coming out with9:30familiarity with ultrasound 40% of9:33residency programs are being trained on9:36butterf on9:37butterfly it's inevitable it's done so9:41Jo giggling because I keep thinking of9:43your past with virtual Care telea Health9:46with InTouch and then with tedoc and9:49trying to sell that technology into9:51Health Systems and into doctor's offices9:54and trying to change their behavior9:55there and I got to ask you what did you9:57learn from that experience I mean like9:59you're you almost reset yourself back on10:01the hype cycle at like beding Edge10:03technology early adoption so like what10:05are you taking from that experience10:07having pushed through virtual care into10:11the you know incumbent established you10:13know care Pathways and that was a slog10:16and I mean I I spoke with you you know10:18during covid and and you were like our10:20man on the street in hospitals10:22desperately trying to stand up virtual10:24care who had not listened to you for10:25years before them um and that then10:28needed it and so now here you are you're10:30with a with a technology that's novel10:32again that requires a behavior change10:34that requires you know all different you10:36know care processes should be built10:37around it so like what are some of the10:39things from your past that you know are10:41coming back up for you that you're10:42applying to this in terms of helping10:44change this Behavior moving forward so10:46that the tech can be adopted you know10:48you're very observant that's what I love10:50about you you know is that you10:52understand everything before you come10:55into a dynamic like this an interview10:57like this um it's the exact same you11:00know that and that's that's what11:01attracted me to Butterfly is there's a11:03very novel uh Hardware technology but11:06there's this concept of a network and11:09concept of democratization of care and11:11also even for that matter the concept of11:15remote care so there like when we11:17there's only 880,000 ultr sonographers11:19in the us but we just talked about11:22millions of doctors and nurses and the11:24ability to help a rural hospital do a11:26scan uh can all be be done through11:29through tele medicine and teleg guidance11:31everything we have 100,000 devices11:33around the world that stream data into a11:37cloud which gives us a very large AI11:40data set and so the ability well first11:43of all you know you have to have a11:45Northstar on what are those behaviors11:47that have to change second of all you11:50have to then invest in those processes11:53that are going to accelerate change and11:55for us it's education and we've we've11:58invested11:59now in scan laab which is a new AI tool12:02that helps di DCT training we have12:05butterfly Academy which is like12:07University of Phoenix where you can do12:09an online course take tests etc etc etc12:12um we have certification training so so12:16it is so Sim That's What attracted me so12:19much to Butterfly because it is this12:22digital Health technology on a rapid12:25Innovation Pace that's changing care12:28that's making it EAS easier to create12:29creater access I mean you know one of12:31the things about butterfly is and very12:33similar to InTouch is you know there's a12:36thousand of these probes in12:39Ukraine um12:42Medics Medics who are on the battlefield12:45that's incredible who are helping12:49triage you know one of the things about12:51butterflies we have one probe that works12:53for all used cases our competitors have12:56to have multiple probes with multiple12:58frequency icies because they're12:59pre-programmed for each of those use13:02cases the ability to bring Health Care13:06where it's needed we've we've trained13:09500 midwives how to do gestational13:13positioning so prior to birth they would13:16set the birth up for success to make13:18sure that the child is in the right13:20position and those midwives are are13:22using this every single day it's about13:25bringing care to where patients are so13:27virtual care is bringing bringing13:29expertise to where patients are13:31ultrasound and and the Butterfly device13:35is empowering people to help people13:38where it happens not to say oh we have13:41an issue uh let's create an appointment13:43let's get you in hospital next Tuesday13:46let's go into a high-cost environment13:49because a lot of these other places13:50around the world don't have access to13:52those13:53environments right let's talk about the13:55US for just a second here and and this13:58is going to be probably like a13:59reoccurring nightmare from your telea14:00health days but payment model you know I14:03mean that's the thing right and that was14:04one of the the big issues you know that14:06that had often kind of held the adoption14:09of telea Health down was that it wasn't14:11reimbursed at parody and then you know14:13the pandemic happen suddenly those14:15restrictions and other restrictions14:17about where originating site all of that14:19stuff are lifted you know and so I'm14:21curious the parallel here with point of14:24care ultrasound like I mean I don't know14:26this but is is that reimbursed the same14:29way that an ultrasound in a traditional14:31room with that the traditional14:33ultrasound tech is reimbursed or is it14:36reimbursed differently because you know14:37how that ends up impacting you know14:39uptake by you know the hospital system14:42or the clinician group who's looking to14:44generate revenue from providing that14:46that source so tell us about the14:47reimbursement model around this so14:50fortunately we do have reimbursement and14:52okay great and and the way it works is14:55there's a higher level of reimbursement14:58to do a cart-based INF facility15:01ultrasound um but that's very it's15:03almost very similar to surgery done in a15:06hospital versus in a surgery center so15:09there's different reimbursement because15:10you have a different cost base of of15:13that environment so in hospital you have15:15to pay for a very expensive cart you15:18have to pay for your facility fee and15:20then you get the The Physician15:21reimbursement uh Physicians can can do a15:25scan and get anywhere between 7 to15:29$120 for a five 10 minute scan so it's15:34when you look at the time it's actually15:37very profitable the the biggest15:38challenge we have with reimbursement15:39today is not the existence of15:41reimbursement is the Comfort level of a15:44doctor to actually feel that they can15:46claim for reimbursement so today in15:49hospitals and in point of care hospital15:52only sees about 35% of the scans they15:55take are actually um going into the EMR15:59into16:00dcom and then being reimbursed for so16:03our challenge is building up their16:05education building up their confidence16:08and then we've also created a middleware16:10that allows us to link probes data16:14images into epic and into dcom and every16:18single preset we have in our probe we16:21have 20 presets so if you're going to do16:23an abdominal scan you do an abdominal16:25preset and then you can document into16:28Epic from your your16:30device and then at the bottom it has all16:32the codes that you could potentially16:34reimburse from so we have everything16:36there the the biggest Missing Link in16:39point of care ultrasound is simply the16:42clinician having the confidence wow did16:45I get it and and and did I and am I16:48confident enough in my decision that I'm16:51now GNA claim reimbursement because16:53there's liability and whatnot tied to of16:55course a diagnosis like if I just pull16:58this and just just scan you I I it's17:01just more information for me but the17:03moment I make a decision and I document17:06that diagnosis and then I ask for17:08payment then that's a higher bar and so17:11so for us you know if you look at the17:14market adoption17:16curve we're still early at the early17:18adopters and we're just now Crossing17:20that Chasm we're just now breaking into17:23the early majority the acceptance17:26hospitals and and clinicians17:28understand that Imaging at the point of17:31care is better for the patient and I17:33think Radiology understands today this17:35isn't a threat because the the the17:37Ultras sonographers in the hospital are17:39at at capacity they can't do anymore and17:42there's a weight list so the ability of17:46of having an intensivist round with17:49ultrasound so they can make a decision17:51right then and there or an emergency17:53doctor having17:54ultrasound to be able to make a decision17:56so they don't have to wait three hours17:58for their scan is just all incremental18:00revenue for the hospital and the health18:03system I think that's so fascinating say18:05a little bit more about some of these18:06some of the ways that you guys have18:08built into the Technology support for18:11those clinicians as they're scanning18:13because this is one of the the really18:14interesting components of this to me is18:16just how much like handholding there is18:18so so to kind of remove that icky18:21uncomfortable factor of am I scanning18:23this right I mean you mentioned the18:24preset so say a little bit more about18:26that technology and how you guys are18:27leaning into Tech in that way in order18:30to help kind of remove any hesitation18:33there might be on did I get a good18:35enough scan to make a diagnosis and then18:37bill for it right well you know every18:40hospital has different credentialing18:43protocols and so it's up to the hospital18:46to decide who is credentialed and who18:49would then have the ability to to to18:51perform whatever under uh under being in18:54their18:55facility what we do is we offer a bunch18:59of different different levels of19:01Education to help them get to that level19:03of comfort and it's up to the clinician19:06in the hospital to determine if they've19:08hit that Comfort level and if they're19:10ready to get there we do everything in19:12the19:13UI where we have actually in the device19:17uh training videos if you're in that19:19preset and you want to know how to19:21position19:22something right then and there you can19:24get that video to help you uh we have an19:27AI tool for19:29practicing where it'll tell you where to19:32put the probe it'll show you what your19:34image should look like and then when you19:36have a live19:38image there are AI labels on the anatomy19:42telling you what you're looking at19:44there's the mro valve there's the sepal19:47there's the there's the aorta here is19:50you know the the the the uh Atrium so19:54the the ability to get more and more19:56comfortable we we have invested Ed in19:59every possible tool and we've invested20:03heavily in going to the residency20:05programs and the medical schools to get20:08early early and early on so whether it's20:11tele medicine whether it's software to20:14review scans of people who've done scans20:17rate those scans and then graduate them20:20to a different level uh all the tools20:22exist what what needs to happen now I20:25have a general practitioner we all do20:28he's my age uh and he drives me up the20:31wall he tells me he's too busy he's20:34telling me I can't learn something20:36new I'm the CEO of butterfly you have to20:39do this I can't come to your office if20:41you're not gonna point of care20:43ultrasound but literally we're dealing20:46with some people who are at the end of20:48their career they don't want to learn20:49something new they're too busy it's not20:51their workflow and that is just the type20:53of change you know the the what's20:56exciting about the business that we're20:57in is we're helping people yeah is Doc21:01and doctors who uh Embrace this are able21:05to help people and they don't have to21:07sacrifice it economically it's very low21:10cost to buy a device and then the21:13reimbursement exists and the workflows21:16make sense but we just have to get21:18people more comfortable all right I want21:20to ask about two things that I'm really21:22excited about and so the first one for21:24you guys is the launch of the butterfly21:26garden so this is like you guys are21:28jumping into the you know AI bandwagon21:30and you're opening up your platform to21:33let developers um develop artificial21:35intelligence technology on top of the21:38the platform that you guys already have21:39tell us a little bit about that like how21:41did where did this idea come from and21:43how do you hope that it will be used to21:46help further the ease of use of the the21:49butterfly devices in practice well that21:51that's you're you've almost answered21:54your question oh great I love when I do21:57that Joe you're you're so switched on21:59it's it's22:00insane the essence of adoption is making22:03it easier and AI tools can make it22:06easier to a uh capture the image and22:10then B interpret the image and these22:12tools are trained on massive data sets22:15and they're really really impactful I22:18mean Bill Gates has given money to GE to22:20Phillips to ourselves Gates Bill Melinda22:23Gates Foundation22:25respectfully because they believe that22:28AI is the bridge like as I said we have22:30to get to that level of comfort we have22:32to get to that level of22:34adoption and AI is a massive accelerator22:38because it dramatically reduces the22:41complexity and so we said to ourselves22:43okay um there's about 150 different22:47companies developing AI applications for22:52Imaging and22:53ultrasound now we can either look at our22:57development and say22:58we're going to hit all of these23:00different things and we're going to23:02develop all this23:04Ai and we're going to compete with those23:07150 companies or we're going to focus23:11our energy on all the things that we do23:13that are unique all the things that we23:15do that no one else can do but then open23:18our platform to those 150 plus Plus+23:22companies and let those products come23:25into our ecosystem why because our23:27customers are going to23:29benefit every time a new app comes in23:31for DVT or for cardiac echo or for23:35vascular access it just makes our23:38platform more valuable and so I can23:40either invest my R&D to try to compete23:44with and replicate things that other23:46people are already doing or I can invest23:48my R&D in things that only I can do and23:52open my platform for everyone else to23:54come in so the way our model model works23:58is we get paid for Access and then we24:02split Revenue with our partners so for24:05us it's a stacking Revenue stream it it24:07it embarrassingly is very simple similar24:10to the app App Store you know where you24:12have these apps come in the platform24:14Apple makes money on them it's it's very24:17simple and and so for us uh what we want24:21to do is accelerate adoption of all of24:24the AI software and we want our24:27customers to not have to choose what24:30platform they have to be on to get what24:33piece of software so we just made the24:36decision we publishing uh a software24:40development kit that makes very easy for24:43someone they put an app in the app store24:46themselves that are their butterfly24:48application you plug a b butterfly into24:51the iPhone you open their app and it24:54works awesome I love the simpl of that24:58now the second thing that I wanted to25:00ask about that got me excited is like25:02the other investment in R&D that you25:05guys have made recently which was in the25:07development of that semiconductor25:08technology that is behind this third25:11generation version of the device and25:14this was cool to me and I'm sure and I'm25:15asking this really for the shareholders25:17who are out there who are like let's25:19talk about this new Revenue stream25:21capability right so let's talk about25:23this new potential Revenue stream here25:26so you guys are looking at licensing out25:29that semiconductor technology talk talk25:31to us about this this idea this was good25:34well again you frame it correctly25:37because the way I looked at this was you25:39know our company has invested a half a25:41billion dollars to develop the25:43semiconductor technology and we're25:45targeting what is today a pretty small25:47Market with a large opportunity and we25:50you know just being a commercial25:52Enterprise we need to be able to grow25:54our Revenue we need to be able to short25:55up our balance sheet so if we have a25:58novel technology that can work in26:01non-competitive environments why26:03wouldn't we monetize that so we made it26:05available and26:07amazingly we've developed two partners26:09so far and we actually have a very26:13exciting pipeline where you know that26:16what what our chip does is not only26:19deliver ultrasound but it's like we have26:219,000 little sensors on the chip it's26:25almost like thinking about 9 th00026:28pixels on a screen that we're looking at26:31and the computer can control the color26:32of each one of those pixels right that's26:34how you see me I see you well we can26:38control the energy out of every single26:40one of those individual M26:43sensors so we can control the beam and26:46how it's26:47formed the normal ultrasound beam is26:50like a flashlight the beam comes26:53straight up what we can do is we can26:56keep the probe still26:58and then we can move the Beam without27:02moving the probe and so it it it's27:06almost like the Star Trek tricorder I27:08don't know you're not old enough you're27:09a young lady no but I've been to27:12enough with tech people in healthcare to27:15have heard about the tri and seen27:17pictures and watched the video yeah so27:20so you can literally like you see the27:22thing go through the body it's not27:23actually too dissimilar to our27:25capability so wow we created a27:27partnership with um Forest27:29neurotech out of Caltech who's creating27:32a brain computer interface and what they27:35are trying to do is is core the bone out27:38of the apex of the skull place an27:41implant into that Apex with our chip27:45then our chip will scan through the27:48entire brain and it'll look at the brain27:52and look at the vascularity inside the27:54brain and27:56see what parts of the brain are deprived27:59of enough of enough blood flow and then28:02deliver energy to those parts to28:05accelerate bread flow so if we weren't28:08willing to share this28:10technology something like that I mean28:13they would have to go and walk the path28:16of creating that type of investment28:18dollar to to create access so what we28:21said was why won't we monetize that so28:25we created a licensing deal we Reed28:28money and everything they sell going28:32forward have our chip in it and we're28:35Partners so incredible and so there's so28:38many other applications there's another28:40application called mandera that has a28:43many robot that attaches into an28:45ultrasound device and you can just touch28:48on the screen boom I want to go there28:50and the robot arm all of a sudden points28:53at it and if I move the probe the robot28:56arm is still still fixing on exactly28:59that point so I can put a needle exactly29:01where I want on the image it it's29:03fascinating so why wouldn't we allow our29:06technology to come into that company so29:09I think we're going to see just like29:12Intel Inside powered by butterfly is29:15going to be a part of of healthcare and29:17other companies are going to leverage29:18our technology and the way you started29:20the question which was perfect our29:22shareholders will benefit oh my God29:24totally they're like probably like yeah29:26I can I can see you know the the the the29:29stock Rising right now as they're29:31listening to this right to God's29:36ears well hopefully hopefully right all29:39right Joel let's wrap this up I mean29:41like I said at the beginning you know29:42you're a friend of the show we've talked29:44a couple times and you know the thing29:46that it's always nice to see somebody29:48that I've interviewed before and29:50especially in a different context and so29:52I just I for to wrap it up I've got to29:54ask you know you had your successful29:56exit you exited in touch $600 million to30:00tedak at the beginning of 2020 you30:02survived the pandemic you live out the30:04contract there you take a year off and30:06now you're back like I mean I had to say30:09I was excited to see you as as the CEO30:12of butterfly because like I said I do30:13see so many parallels between your your30:15lived experience and in teleah health30:17and now you know taking this new30:19technology into hospitals but here from30:21you you know what brought you back into30:23Health Tech I mean this is not an easy30:26Road well you know I guess have you ever30:29heard the story of the frog and the30:31Scorpion30:33no the Frog's about to cross the river30:35and the Scorpion says give me a ride30:37frog says why would I do that you're30:39gonna you're G to sting me scorpion says30:42why would I do that I would die with you30:44so he says okay he hops on they go30:46across halfway across the river the Frog30:48feels that uh the F he feels the30:51stinging in his back and he goes30:53scorpion why would you do that we're30:54both gonna die he goes well look I'm a30:56scorpion you know you are what you are30:59you are what you are I'm a med tech guy31:03um I I tried to stay home I tried to31:06spend time on the31:07ranch but uh I love it and and when31:11butterfly Came31:12Calling just like I said before I I saw31:15so many parallels in in digital and
